2. Download our health questionnaire HERE & fill out as much of it as you can. Note: The more information we have about you, the better we can design a program that best suits your health concerns, fitness goals & lifestyle.

3. Fax or mail it back to us ASAP so we can get you started:

  Note: We recommend our Introductory Starter Package which includes 3 personal training sessions and a complimentary 90 minute Fitness Assessment (normally $195 but you get it FREE when you sign up for this package or a program). This is the basic package that will provide you with a specific exercise/wellness program to launch yourself onto your way to wellness!
